In order to be eligible to attend camps, you
must:
| 1) Be between 5 yrs. and up to 18 yrs. of age. |
| 2) Have been diagnosed with Diabetes by a doctor. |
| 3) Complete the required application and print out. Fax or snail mail the form to the designated person. |
| 4) Timing is everything! Please make certain the forms(Release and Application) are mailed by the date listed for your camp. Failure to do so will prohibit your child's admittance to camp. |
